So let me just tell you what I'd tell a friend who came into the pharmacy and asked.

The basics, quickly

Tirzepatide — sold as Mounjaro — is a weekly injection from Eli Lilly. It was built for Type 2 diabetes, but the weight loss in trials was significant enough that it got approved for obesity too. One pen, once a week, same day every week. That's the routine.

What separates it from similar medications is that it activates two receptors — GIP and GLP-1 — rather than just one. That dual mechanism is why the numbers tend to be stronger. It's not marketing language, it's just how the pharmacology works.

Who it's genuinely for

This is where I want to be straight with you, because social media has blurred the lines badly.

Mounjaro is clinically indicated for adults with Type 2 diabetes, or for people with a BMI of 30-plus, or 27-plus if there's a related condition alongside it — things like hypertension, prediabetes, or obstructive sleep apnoea. A doctor needs to assess you, check your bloods, review your full medication list. In the UAE it's prescription-only, full stop.

If you're a healthy person who wants to lose a few kilos for Ramadan — this isn't the right tool. Not because I'm being preachy, rather they have to be monitered closely.

Nausea is the big one. Not severe for most people — more of a persistent low-grade unsettled stomach, particularly in the first two to three weeks after each dose increase. It usually settles. Eating smaller meals helps. Avoiding fatty foods in the first few days after your injection helps more.

The hunger reduction is gradual but noticeable. Patients describe it less as "not being allowed to eat" and more as genuinely not feeling the pull toward food the way they used to. Portions shrink without a fight.

You start at 2.5mg weekly. After four weeks your doctor reviews and may increase to 5mg. The ceiling is 15mg, though most patients find their effective dose somewhere in the middle. Never self-adjust — that's your doctor's call, based on how you're tolerating it.

The cost- no sugarcoating

It is AED 1,734 the standard four-pen pack. MoHAP regulates this so the price is consistent at licensed pharmacies across the UAE.

Over six months that's roughly AED 10,500. It's a real financial commitment. I always tell people to factor this in before starting, because stopping mid-treatment isn't ideal — weight can return when the medication is discontinued, especially without solid lifestyle changes alongside it.

Insurance is a grey area

For weight loss? Most plans won't cover it — it's classified as non-essential by the majority of UAE insurers in that context. For Type 2 diabetes management, coverage depends entirely on your specific policy. Call your insurer before you fill the first prescription, not after.

A few things that matter clinically

If you're on insulin or a sulphonylurea alongside this, your blood sugar can drop lower than expected. Your doctor should review your doses before you start — this isn't optional, it's important.

Mounjaro is not safe in pregnancy. The data isn't there yet, and Lilly is clear about this. If there's any chance of pregnancy, contraception needs to be in place throughout treatment.One more thing — and I can't stress this enough right now. Counterfeit GLP-1 pens have been circulating in the region. Only buy from a licensed UAE pharmacy with a valid prescription in hand. If a price seems too good or a source seems unofficial, walk away.

Does it work?

For the right patient, the evidence is genuinely strong. People hit different response profiles — some lose 10% of body weight, others considerably more. What I've seen in practice is that the people who do best are the ones who also sort out their eating habits and stay in regular contact with their doctor.

It's not a background medication you take and forget about. It needs attention, monitoring, and honesty with your healthcare team.
