Description
Janumet XR 50mg/500mg is a fixed-dose combination therapy engineered for the management of type 2 diabetes mellitus. It integrates Sitagliptin (a dipeptidyl peptidase-4 inhibitor) and Metformin Hydrochloride Extended-Release (a biguanide) to provide dual-pathway physiological support for blood glucose regulation by improving insulin secretion, reducing hepatic glucose production, and enhancing insulin sensitivity.
Dosage & Administration
- 1 to 2 tablets per day, as determined by clinical assessment.
- Administer orally as a single dose.
- Timing: Must be taken once daily with a meal, preferably in the evening, to improve gastrointestinal tolerability and maintain steady-state release.
- Swallow whole: Do not split, crush, or chew. The extended-release polymer matrix must remain intact to ensure controlled drug delivery over 24 hours.
- Co-administration: If prescribed two tablets, both must be taken together at the same time.
Safety & Storage
- Boxed Warning (Lactic Acidosis): Metformin accumulation can lead to fatal lactic acidosis. Risk factors include renal/hepatic impairment, age $\ge$ 65, and excessive alcohol consumption.
- Procedural Note: Discontinue Janumet XR 48 hours prior to and after any radiologic procedure involving iodinated contrast to prevent acute renal failure.
- Adverse Reactions: Common side effects include diarrhea, nausea, headache, and upper respiratory tract infection. Monitor for severe abdominal pain (potential pancreatitis).
- Storage: Store in the original container below 30°C. Keep the bottle tightly closed to protect from moisture and light.
