So. What actually happens at each stage?

Mounjaro (tirzepatide) works through two pathways at once: GIP and GLP-1 receptors both fire, which is the mechanical reason it outperforms single-receptor drugs in head-to-head data. It starts at 2.5mg, injected subcutaneously once a week. After 4 weeks your doctor bumps it to 5mg, then by 2.5mg every 4 weeks from there. Adults cap out at 15mg. Kids aged 10 and older cap at 10mg.

The FDA-approved climb runs from 2.5mg to 15mg in 2.5mg increments, with a hard floor of 4 weeks between each change: so 20 weeks total if you go straight through without pausing. Mounjaro 2.5mg: The starting dose

Call it a warmup round.

The 2.5mg phase runs 4 weeks and isn't about blood sugar at all: your gut has no frame of reference for tirzepatide yet, and jumping straight to a therapeutic dose would hit most people with nausea, diarrhea, and cramping bad enough that they'd want to quit, so the initiation dose gives your GI system 4 weeks to adapt before the real work begins. Nausea rates at 2.5mg are the lowest in the whole protocol. Most patients describe these 4 weeks as uneventful, which is exactly the point.

Mounjaro 5mg: First therapeutic dose

Week 5. Glycemic work begins. About 39% of patients report some GI event at this level: nausea leads at 12-18% in the type 2 diabetes trial data, followed by diarrhea and appetite reduction, and while these are dose-dependent effects that typically back off as your body adjusts over the 4-week window you're at 5mg, a real subset of people find them persistent enough that their provider delays the escalation to 7.5mg by another month.

Community tip: 

Smaller meals, protein first, during the first couple weeks at a new dose helps a lot of people manage nausea. Not in the FDA label: just something patients repeatedly report.

Mounjaro 7.5mg: Second step up

Same deal as every other escalation: 4-week minimum at 5mg, then your doctor decides whether the numbers justify going higher.

GI event rates tick up again at 7.5mg. If tolerability at 5mg was rough, there's a real conversation to have about whether stepping up right at 4 weeks makes sense or whether waiting longer is smarter.

Mounjaro 10mg: Approved. Not at MedX.

Mounjaro 12.5mg: When 10mg isn't enough

Mounjaro 15mg: The maximum adult dose

In the SURPASS-2 trial (NEJM), 15mg patients lost 12.4 kg over 40 weeks. Semaglutide 1mg patients lost 6.2 kg. HbA1c dropped 2.30% on 15mg: the 2.30% figure you'll see cited everywhere for tirzepatide comes from this arm of this study. GI events at this level run about 49%, the highest in the schedule.

One detail that catches people off guard: every KwikPen injects 0.5 mL regardless of strength. 2.5mg, 15mg, anything in between: same 0.5 mL. Managing the escalation: what the side effect data actually says

The GI rate numbers are worth sitting with: 39% at 5mg, 46% at 10mg, 49% at 15mg. That's nearly half of people on the top dose reporting some kind of GI event. Nausea and diarrhea are the most common. Vomiting shows up in 5-9% of patients, constipation in 6%, dyspepsia in 8%.

Slow down. Really.

These are dose-dependent. Slower titration helps.

The prescribing information sets a 4-week minimum between steps, not a 4-week maximum, and plenty of real-world prescribers push that window out whenever a patient is struggling to tolerate a given dose, because there's no clinical penalty at all for staying at 7.5mg for 8 or even 12 weeks if that's genuinely what's required to keep someone on the medication long enough to see the therapeutic benefit. That call lives entirely with your doctor.

Sitting longer at each dose before stepping up correlates with better GI tolerance, according to what patients report. Not a clinical directive: just a pattern worth raising with your prescriber.

 

Worth flagging: tirzepatide dosing errors showed up in the FDA's Adverse Event Reporting System (FAERS) at 8 times the rate in 2024 versus 2022, making them one of the most consistently reported adverse events for this drug, and while most errors are minor timing mistakes, double-checking your current dose with your pharmacist each time you move to a new strength is the kind of 30-second habit that catches a surprising number of them.

One more thing: Mounjaro is FDA-approved for type 2 diabetes, not weight loss. Weight reduction is a well-documented secondary effect, but the weight management indication belongs to Zepbound: same molecule (tirzepatide), different brand, different label. Off-label use for weight loss is a conversation with your prescriber, not something to self-direct.

Missed a dose

96 hours is the cutoff.

Miss it? Check the clock.

If you're within 4 days of your scheduled injection, take it. Resume your normal weekly schedule from there. If you're past 96 hours: skip it. Pick back up with your next scheduled dose. Don't double up to make up for the miss; that's not how this medication works and it creates real risk.

You can permanently shift your injection day if your schedule changes, as long as you've gone at least 3 days since the last dose, and this flexibility applies at every strength from 2.5mg through 15mg so it doesn't matter which point in your titration you're at when you need to make the adjustment. When timing gets confusing, your pharmacist is a faster call than you'd think.

Storage and injection basics

2 to 8 degrees Celsius: that's your fridge target for unused pens. Once you break the seal on a pen and start using it, you've got 21 days at room temperature (up to 30 degrees Celsius) before it has to go.

Injection sites: abdomen, thigh, or upper arm.

Rotate every injection. If insulin is also part of your regimen, the same body region is fine for both but not the exact same spot: leave some distance between injection sites. What is the highest dose of Mounjaro?

Adults: 15mg. Kids: 10mg.
